PLEASE READ THESE INSTRUCTIONS CAREFULLY BEFORE RESPONDING...

Please complete the following assessment based on your service-learning or community-based research experience this semester. You should read each item and go with your strongest response then move on. There is no right or wrong answer; what is important is that you answer as honestly as possible. This survey should take you approximately 30 minutes to complete -- but keep in mind that your session will "time out" and you will lose your responses if you do not complete it and hit "finish" within 1 hour of opening this form. Note: If you are currently in MORE THAN ONE service-learning or community-based research class, please complete ONE assessment survey for EACH class.

Your assessment needs to be submitted to us by the beginning of the final examination week. Your feedback is essential to the success of ASU's Service-Learning & Community-Based Research Program. However, your responses will never be connected to your personal identification. Class participation not individual answers will be provided to professors for teacher evaluation purposes.
